Public-safe modernization case study

The Decision Record Is Part of the System

Published 12 October 2026 · 08:28 MDT

Modernization becomes durable when the evidence, authority, assumptions, and acceptance behind important choices remain available after meetings, staffing changes, and vendor transitions.

Capture the why

A decision log should record the question, alternatives, evidence, constraints, approving authority, date, dissent or uncertainty, and conditions that would trigger reconsideration.

Connect the chain

Each decision should link to the originating need, requirement, authoritative source, control, test, acceptance evidence, release, and accountable service owner.

Minimum decision record

Govern through evidence

  1. Identify which choices require formal governance and which can remain team-level decisions.
  2. Use a consistent record template and authoritative repository.
  3. Require traceable evidence before approval or acceptance.
  4. Review unresolved assumptions at defined delivery gates.
  5. Preserve superseded decisions so future teams understand why the system evolved.

Executive takeaway

Architecture diagrams describe what was built. Decision records explain why it deserves trust. Treat both as first-class modernization assets.
